Program Assistant

General Scope of Duties:

Provides services and supports to individuals at their residential settings according to each person-centered plan. Recognizes natural learning opportunities and uses those to teach independent living skills, enhancing individuals’ independence. Maintains the home within established standards, including health and safety guidelines, protection of individual rights, and fiscal parameters.

Responsibilities:

1. Delivers services and supports as outlined in each individual's Person-Centered Plan ensuring that activities and supports reflect individual preferences.
2. Communicates regularly with the Group Home Manager, QMRP, nursing, and other team members regarding progress towards outcomes, health and safety, and other issues that may arise.
3. Assists individuals in maintaining their home, reporting needed repairs or modifications to the appropriate parties within established timeframes.
4. Recognizes individual differences and adapts teaching/ interacting style to best support the person's needs.
5. Advocates for individuals through knowledge of personal references, protection from abuse and neglect or other rights violations, and supporting the development of personal relationships.
6. Provide required documentation regarding program delivery, medication administration, financial records, and all other requested documentation.
7. Perform other related duties as assigned.

Knowledge, Skills and Abilities:

1. Must communicate effectively, both verbally and in writing.
2. Mobility to perform assigned duties in various environments including bending, kneeling, standing, repeated lifting.
3. Ability to perform CPR, First Aid, and aggression management.

Working Environment:

Work environments may vary to include residential setting, and community settings. May be required to work extended shifts and for be called upon for emergencies.

Qualifications:

1. Must be at least 18 years of age.
2. Posses a High School Diploma or equivalent (GED).
3. Possess a valid driver’s license and automobile insurance.
4. Complete and maintain DSP certification, CPR, and First Aid Certificate and be authorized to pass medication once properly trained.
5. Must be able to lift up to fifty pounds.
